Abstract (Croatian)

U dostupnoj literaturi postoje različiti pristupi u rješavanju i tumačenju trzajnog gibanja glave i vrata. M. D. Freeman i suradnici navode broj od preko 10000 radova vezanih uz pojavu trzajnog gibanja glave i vrata. Bez obzira što je navedeno gibanje bilo predmetom brojnih istraživanja i rasprava ono je ostalo još uvijek nejasno i nedovoljno istraženo. U radu su provedena mjerenja koja su za cilj imala utvrditi kinetičke značajke gibanja. Također su definirana različita gibanja glave i vrata, a za koja se smatralo da bi mogla pridonijeti utvrđivanju podataka pri tumačenju trzajnog gibanja glave i vrata. Karakteristična gibanja koja se pojavljuju u glavi i vratu subjekta koji je u prometu bio izložena udaru sa stražnje strane su oponašana na dobrovoljcima (u bezopasnim opterećenjima) koji su bili snimani stereofotogrametrijskom metodom, uređajem za multifaktorsku analizu gibanja Elite. U istraživanju je sudjelovalo 50 ispitanika, koji po vlastitom mišljenju nisu prethodno imali nikakva oštećenja vratnog dijela kralježnice. Također su svakom snimljenom subjektu određene osnovne antropomjere. Primjenom temeljnih metoda mehanike, harmonijske analize, antropometrije, statistike i biomehanike definiran je prvi puta harmonijski model glave i dijela vrata dime je postignut model referentnog čovjeka što u dostupnoj literaturi nije dosad na takav način obrađivano. Za ovakav model se pretpostavilo da je pretežno neovisna kinetička jedinica bez obzira što je on u stvari čini kinematičku cjelinu zajedno s trupom. Razlog tome je što u dinamičkom smislu otpor rotacije glave na atlasu značajno manji od sveukupne tromosti glave i vrata s obzirom na koordinatni sustav smješten na donjem rubu vratne kralježnice. Na temelju modela glave i dijela vrata, te snimljenih trodimenzijskih koordinata karakterističnih točaka svakog pojedinog gibanja slijedila je kinematička analiza promatranih gibanja. Analizom su utvrđene orijentacije i položaji središta segmenata modela, putanje središta masa glave i dijela vrata koji sudjeluje u gibanju, te promjene središnjih dinamičkih momenata tromosti s obzirom na odabrani koordinatni sustav. Pored toga je utvrđeno da je promatrajući ravninsko gibanje glave i vrata sastavljenog od translacije i rotacije značajno izražena prvo rotacija glave oko prvog kralješka — atlasa. Utvrđeni rezultati omogućuju usporedbu mehaničkih veličina što su mjerenjem proistekle na subjektima čija se gibanja glave i vrata mogu smatrati "normalnim" i gibanja koja bi izvodili subjekti s oštećenjima vratne kralježnice.

Abstract

In the available literature there are various approaches to solving and explaining whiplash movement of head and neck. The M. D. Freeman et al mention a number of more than 10,000 works related generally to the phenomenon of whiplash movement of head and neck. Regardless of the fact that the mentioned movement has been the subject of numerous studies and discussions, it is still lacking clarity and research. In this work measurements have been carried out with the aim of determining the kinematic magnitudes of movement. Different movements of head and neck have also been defined, which were considered as being useful in determining the data for the interpretation of whiplash movement of head and neck. The characteristic movements that occur with head and neck of subjects hit from the back in traffic accidents has been simulated on volunteers (with safe loads), recorded by stereophotogrametric method, with the system for multifactor analysis of movement Elite. 50 examinees took part in the research, and according to their own opinion had had no previous damage of the cervical vertebra. Also, for every recorded subject the basic antropo-measures have been determined. By application of the basic methods of mechanics, harmonic analysis, antropometry, statistics and biomechanics, the harmonic model of head and a part of the neck have been defined for the first time, thus obtaining the model of a reference human, not analysed in the available literature up to now. This model was assumed to be predominantly an independent kinetic unit regardless of the fact that it actually represents a kinematic unit together with the body. The reason for this lies in the fact that in the dynamic sense the resistance to the rotation of the head on the atlas is significantly less than the overall inertia of head and neck with relation to the co-ordinate system located at the bottom edge of the cervical vertebra. On the basis of the head and part of the neck model, and the recorded three-dimensional co-ordinates of characteristic points of each movement, kinematic analysis of the recorded movements followed. The analysis was used to determine the orientations and locations of the model segment centres, trajectories of the mass centres of the head and part of neck which participate in the movement, as well as changes of central dynamic inertia moment regarding the selected co-ordinate system. Moreover, it was determined that by studying the plane movement of head and neck made of translation and rotation, first the rotation of the head around the first cervical vertebra - atlas is significantly pronounced. The determined results allow comparison between mechanical values obtained by measurements on subjects whose movements of head and neck may be considered as "normal" and the movements produced by subjects with damaged cervical vertebra.
